How to make a referral

Who can refer?

We accept referrals from GPs, health care professionals, schools and other local organisations working in the community. Parents/carers can also self-refer themselves or another family member directly into our service.

Referral criteria

Please ensure the person you are referring:

Child or young person criteria:

  • Live in Blacon, Lache, or Kingsway, or attend school in Blacon or Lache or the district of Kingsway.
  • Is from a low-income household and unable to afford private therapeutic services.

Adult criteria:

  • A parent/carer (or someone who may become a parent/carer in the future) and, lives in Blacon, Lache or Kingsway.

OR

  • Registered to a GP in Blacon or Lache, from a low-income household and unable to afford private therapeutic services.

 

Clients must meet all of the above criteria to be eligible for our service.

 

Making a referral 

To make a referral for someone or in case of self-referral for yourself or another family member please:-

  1. Complete the online referral form
  2. If the person you are referring is under 18 years of age, please download and complete a consent form and upload this to the online referral form (referrers only).
  3. For a primary school age child, please could the school referrer or parent (in case of self-referral) complete a parent or teacher SDQ evaluation.

Safeguarding

We come into contact with children/young people and/or vulnerable adults through the delivery of our counselling services and general contact. We take seriously the welfare of all children/young people and/or vulnerable adults that we work with and our safeguarding policy seeks to ensure that Platform for Life undertakes our responsibilities with regard to their protection and responds to concerns appropriately.
